Transcript Requests
Each and every school to which you apply will require an official transcript to be sent as part of the application. Please note that you must fill out and submit the Transcript Request Form for EACH of these schools. Official transcripts will be mailed directly to the college. Some colleges require that an additional form be sent with the transcript, so check your application packet and submit the form with appropriate instructions to the College Counselor.
Deadlines:
Students must be very careful to comply with all college admission office deadlines. Failure to comply may result in failure to gain admission.
Counselor Recommendations
The Secondary School Counselor Report form (found on the common application) should also be submitted to the College Counselor along with the Transcript Request Form. Please provide a copy of your resume/activity sheet as well. All this paperwork will then be sent directly to the college from the College Counseling office.
Teacher Recommendations
The student should submit the following to the teacher:
• A completed Teacher Recommendation Form
• A resume/activity sheet
• A stamped and addressed envelope, full legal name, social security number, (for non-Common Application Schools only)
Teachers will mail these directly to the college. It is a good idea to check with the teacher to see if the letter was sent, and to show your appreciation. Ultimately the responsibility is yours.
Mid-Year Reports
Some schools require a mid-year report. It is the student’s responsibility to know which school requires this report. After completing all the relevant questions, give this form to the College Counselor with envelopes addressed to each institution that requires a Midyear Report.
Final Transcripts
The college that you plan to attend will require a final transcript to verify your graduation. Final transcript requests will be announced in the Spring (April/May). It is your responsibility to tell the College Counselor where you need your final transcript sent.
