Requirements for Participation

 

Academic Eligibility


Students must be in good standing academically (which means they have failed no classes the previous marking period) in order to participate.

Updated Physicals


Each student must have an up-to-date physical on file in the Nurse’s Office.  The date of the physical must be within 13 months of the start of the current sport season.

Permission Slips


Each student must have a permission slip filled out and signed by themselves, signed by their parent/guardian, signed by the school nurse, and turned into the Athletic Office before the first day of tryouts/practice.  Permission Slips can be picked up in the main office of the high school or Annex Building during school hours. 

User Fee


The user fee must be turned into the Athletic Office before the start of each sport season.  Checks should be made out to the school, and the student’s name and sport should be written on the check.  User Fee Amounts

Football, Soccer, Crew, Volleyball, Basketball (7th Grade and Above), Baseball, Softball, and Lacrosse:  $125

Cheerleading, Cross Country, 5th/6th Basketball, and Track and Field:  $100

Ice Hockey and Swimming:  $200

Concussion Policy


Every Mystic Valley athlete and a parent/guardian must do the following:

Take the NFHS Concussion in Sports Course by clicking on this link http://www.nfhslearn.com/electiveDetail.aspx?courseID=15000.  

After taking the course, you (parent and student) must print the following link (Concussion Course Completion Form), read it, sign it, and turn it in with the Permission Slip before the start of the season. 

If any one of the requirements above is not fulfilled, permission to participate in athletics will not be granted.
