Requirements for Participation
Students must be in good standing academically (which means they have failed no classes the previous marking period) in order to participate.
Each student must have an up-to-date physical on file in the Nurse’s Office. Physicals are good for 13 months from the date of the physical exam. They do expire after 13 months and a new physical will be needed in order to continue participation.

We are only accepting online registrations. You must register through FamilyID.
We are no longer collecting Permission Slips. If you have any questions about online registration please call 1-888-800-5583 x1.
The user fee must be turned into the Athletic Office within a week after the start of the sport season. Please wait until after teams are selected to turn in your fee.
Checks should be made out to the school, and the student’s name and sport should be written on the check. User Fee Amounts
Football, Soccer, Crew, Volleyball, Basketball (7th Grade and Above), Baseball, Softball, and Lacrosse: $125
Cheerleading, Cross Country, 5th/6th Basketball, and Track and Field: $100
Ice Hockey and Swimming: $200
Every Mystic Valley athlete and a parent/guardian must do the following:

If any one of the requirements above is not fulfilled, permission to participate in athletics will not be granted.
